Job Description Primary Duties & Responsibilities: Performs and documents assessment of the patient/family (biophysical, psychosocial, developmental, cultural, spiritual and environmental); collects and #analyzes #data from the medical record in order to coordinate patient care. #Directs and/or provides patient care, which may include administering medication, performing procedures and monitoring patient status in accordance with approved nursing practice. Provides appropriate counsel and education to patients/family regarding disease process and symptom management.
