Your Responsibilities Will Include: Conducting assessments of patients’ physical and emotional needs Must be willing to drive to patient homes Implementing and updating personalized care plans Providing guidance to patients and their families on end-of-life care and disease processes Supervising care provided by team members Documenting clinical notes and participating in interdisciplinary meetings What We’re Looking For: Current Arizona RN License in good standing Minimum of 1 year of hospice experience A warm, empathetic approach to patient care Willingness to participate in field visits and team meetings Adaptability to changing patient needs If you’re ready to bring your caring heart and nursing skills to our team, we can’t wait to meet you!
