Here, you will work alongside a passionate and dedicated team, receive endless support and appreciation, and build a rewarding career with one of the most respected healthcare organizations in the world.   As a Transplant Coordinator II, you will coordinate the care of patients seeking candidacy for, or receiving, solid organ transplants—including heart, lung, heart-lung, liver, kidney, pancreas, vascularized composite allograft, multi-organ, or bone marrow transplants. In collaboration with a multidisciplinary team, you will enhance patient care by building relationships through education, guidance, and individualized support. Serving as an integral member of the transplant program, you will help deliver exceptional care to patients experiencing end-organ failure or disease. This role also includes participation in the training and onboarding of new personnel and requires the ability to coordinate patient care across multiple phases of the transplant process. This is a regular PRN position, with caregivers working Monday through Friday between 8:00 a.m. – 5:00 p.m. A caregiver who excels in this role will: Assess the potential transplant candidate or the transplant recipient with the transplant physician and collaboratively determines the need for additional assessment or intervention. Provide education to patients and caregivers regarding all phases of the transplant process to provide optimal outcomes following transplant. Facilitate the scheduling of studies or procedures utilizing fiscal responsibility for patient and hospital costs. Advise referring physicians in the care of patients in collaboration with surgical and medical teams. Complete required documentation according to Organ Procurement Transplant Network (OPTN) guidelines including listing, status changes and post-transplant follow-up information. Review and collate medical information received from referring physicians for patients seeking or undergoing transplant evaluation. Serve as the primary contact for peri-transplant evaluation and post-transplant care for physicians and patient/caregiver and/or payors seeking information. Coordinate all activities related to organ acquisition including evaluation of the patient workup information and/or post-transplant care. Be available by pager and by phone in order to provide immediate assessment and delivery of care in a timely and cost-effective manner. Facilitate plan of care and symptom management per physician recommendations Provide nursing care for patients before and after solid-organ transplantation along with the continuum of inpatient and outpatient care. Reference clinical data and prepare abstract and presentation for the multidisciplinary selection committee to determine transplant decision and plan of care. Coordinate with hospital unit personnel in the discharge of a post-transplant patient and/or living donor patient in applicable transplant programs. Monitor post-transplant patient status (e.g. physical, emotional, complications, rejection, infection) and communicates to provider appropriately. Assess recipient compliance with medications and treatment plan and collaborate with transplant team to initiate changes in medical therapy plan as indicated. Participate in the onboarding and training of Transplant Coordinators. Other duties as assigned. Minimum qualifications for the ideal future caregiver include: Bachelor’s degree in Nursing (BSN) Current state licensure as a Registered Nurse (RN) Basic Life Support (BLS) through American Heart Association (AHA) or American Red Cross Certification One year of experience in acute care, medical/surgical, or ambulatory setting within a hospital environment Two years of experience working directly within a transplant program/center Preferred qualifications for the ideal future caregiver include: Bachelor of Science in Nursing (BSN) Physical Requirements:   Requires full range of body motion, manual finger dexterity and eye-hand coordination.
