What's Available: 0.8 FTE (32 hours per week)  Available to regularly work scheduled hours; must be available for off-shift, weekend & holiday coverage. What you will do: Travel in 30-mile radius to patient homes to assist with basic care for residents in the areas of nutrition, elimination, comfort, mobility and safety Works collaboratively with the health care team members to effectively communicate and document accurate information.
