Here's what you can expect as a Hospital Outreach Manager at DaVita: The goal of this role is to ensure DaVita is the provider of choice to hospitals. This teammate will engage renal discharge planners and case managers in an effort to build and strengthen relationships. Their focus is to ensure a smooth and seamless experience in support of newly diagnosed and hospitalized dialysis patients to ensure a successful transition to the outpatient setting. This is a hybrid role.
